A birth certificate copy or a cancelled Indian passport are required for all former Indian citizens.

Please send the following:

  1. A valid passport.
  2. Complete Online Application.
  3. 2 passport photos, color and 2″ x 2″. (Note: photo must be taken without eye glasses.)
  4. Copy of Driver’s License.
  5. For tourists applicants, copy of itinerary or airline tickets.
  6. For business applicants, a business letter on your company’s letterhead.
  7. For business applicants, a Letter of Invitation.
  8. All minors (age 17 and under) must provide a copy of their birth certificate.
  9. Completed The Passport & Visa Company Order Form
